Transitions to Adulthood Team

Getting started on your own can be difficult, but for a young person with mental illness, the transition can be overwhelming. Transitions to Adulthood provides a wide range of opportunities for young adults aged 16-22 to acquire knowledge and skills that will enable them to find a job and a place to live as well as how to access the resources they need to live successfully in our community.

Transitions staff have collaborated with the University of Kansas School of Social Welfare to pilot the Strengths Model of Case Management with transition-aged youth for the first time. Staff have undergone an extensive process of training and review by staff from KU and have been certified to provide the Evidence Based Strengths Model of Case Management.
